• We DO NOT accept HMO plans or Blue Select; So you are welcome to schedule here online.
  
• If you have any other insurance (e.g., Cigna, United Healthcare, Aetna), you are Out-of-Network and will be charged Self-Pay rates: So you are welcome to schedule here online.

• New Patient Visit: $350
  
• Follow-up Visit: $250
  

Please note: The estimated full amount of the service is required prior to your scheduled appointment. Someone will contact you within 24 hours of booking to collect this payment. If we're unable to reach you to collect payment, your appointment will be cancelled.

Attention:  Dr. Reyes is only accepting telemedicine patients through the virtual telemedicine platform, Dr. Alex Reyes LLC.  You will be required to pay at time of booking and Dr. Alex Reyes LLC does NOT bill any insurance carriers.  You are welcome to use your insurance for any lab testing, or other doctor’s orders, just like any doctor’s office.
